Citizen Toxie: The Toxic Avenger IV is a 2000 American superhero comedy film. Lloyd Kaufman directed it, and Trent Haaga wrote the script. It is the fourth movie in The Toxic Avenger series. Although it is the third sequel to the original film, Stan Lee's opening narration states that Citizen Toxie is the real sequel to the first movie, ignoring what happened in the two sequels that came before it.

Lore & Background
The plot follows the Diaper Mafia, criminals dressed as infants, who commit a mass shooting at the Tromaville School for the Very Special. The Toxic Avenger (Toxie) intervenes, kills the gang members, and later impregnates his wife Sarah. An explosion transports Toxie and two students to Amortville, a mirror universe. Meanwhile, the evil Noxious Offender (Noxie) appears in Tromaville and begins murdering citizens. Toxie encounters alternate versions of characters, including a scientist named Chester (the alternate Lardass) and Evil Kabukiman. Noxie becomes mayor of Tromaville and impregnates Sarah. Toxie eventually returns to Tromaville by clicking red shoes and saying 'There's no place like Tromaville.' He fights Noxie while their babies fight inside Sarah's uterus, ultimately killing Noxie. Sarah gives birth to twins—one fathered by Toxie, one by Sgt. Kabukiman.

Stephen Holden of The New York Times noted that while the humor is very funny, the comic excess is exhausting. Den of the Geek criticized the film for turning nasty and including pointless celebrity cameos. The film was released on DVD on March 18, 2003, with deleted scenes, commentary tracks, and a documentary. A fifth installment, Toxic Twins: The Toxic Avenger V, was announced but as of 2025 has not entered official production, with uncertainty due to a planned reboot in August 2025.
